If you signed up for email or event updates from February 1, 2022, through January 31, 2023 on the USC Arcadia Hospital website, you are a Class Member.
If you are a Settlement Class Member and wish to receive a cash payment then you must submit a Claim Form by December 7, 2026.
As a Settlement Class Member, you are eligible to make a claim for a Settlement Payment—a pro rata Cash Fund Payment (equal payment paid to all Participating Settlement Class Members who submit a timely and valid Claim Form)—to be paid from the Net Settlement Fund. You do not need to submit this Claim Form to receive CyEx Privacy Shield Pro; your unique activation code and enrollment instructions are included in the Notice you received.
The amount of each cash payment will depend on the number of approved Claims and the deductions authorized by the Settlement Agreement. Cash payments will be distributed only if the Court grants Final Approval and the Settlement becomes effective.
To start your online claim form, you must login with your LoginID and PIN. You can find your LoginID and PIN on the Notice that was sent to you. If you are unable to locate your ID and PIN, please contact us by emailing info@USCArcadiaHospitalSettlement.com. Please request your LoginID and PIN by providing your full name and mailing address.

The deadline to submit a Claim Form online or by email is December 7, 2026. If you are mailing your Claim Form, it must be mailed with a postmark date no later than December 7, 2026.
